Intake Specialist I

FreedomCare · Remote (IN / US)

  • Educate patients and caregivers on the application and enrollment process for FreedomCare's self-directed home care program.
  • Be the initial point of contact for calls from prospective clients, caregivers, and pending enrollees.
  • Manage a high open caseload until enrollment is complete; consistently meet monthly completed Start of Care (SOC) goals.
  • Communicate regularly with Case Management agencies on patient authorizations; navigate multiple systems to determine eligibility.
  • Maintain data input, scheduling, document uploading, and initiate enrollment confirmation/rejection communications.
  • Requires high-volume inbound/outbound call center experience, poise with clients, strong typing, and computer proficiency.
  • Remote with potential travel for meetings into the nearest FreedomCare hub; in-house training and career development.
